Before removing the device to test or position equipment, the work area must first be cleared of all tools and materials, and all employees must be safely removed from the danger zones.
Each authorized employee must place their own personal lock or tag on the energy-isolating device. If the device doesn't allow for multiple locks, a hasp or lock box is used so everyone can attach their individual lock.
The onsite employer and the contractor must inform each other of their respective LOTO procedures, and the employer must ensure their employees understand and comply with the contractor's energy-control program.
Employers must ensure an orderly transfer of LOTO protection. The incoming shift members must personally verify that the machinery is effectively isolated and de-energized before beginning work.
Management must verify the employee is not at the facility, make reasonable efforts to contact the employee at home, document the removal, and ensure the employee is notified of the removal before they resume work.
