Consider physical changes that will make the incident impossible perhaps a sidewalk and railing to keep pedestrian through the line, consider training options and additional safety equipment like warning signs. List everything that will prevent accident from happening again. It is important to remember that there’s no perfect solution to an accident nor there only one solution, only the ones that can be implemented and will prevent reoccurrence.
